1. Print a copy of the myMAX Online Branch application in PDF format. If you don't have Adobe Acrobat Reader, you may download it for free using the link at the bottom of this page.

2. Complete the Joint Owner section of the application if there is a joint owner on your account who also would like access to the account online. The joint owner also needs to sign the application.

3. Sign the application. You may mail the application to MAX, Attn: myMAX Service Center, P.O. Box 244040, Montgomery, AL 36124, or bring it to any of our convenient MAX branches. Please note that mailed applications must be notarized. You may download a notary form here.
