Online Banking Application

1. Download and print a copy of the Online Banking application in PDF format.

2. Complete the Joint Owner section of the application if there is a joint owner on your account who also would like access to the account online. The joint owner also needs to sign the application.

3. Sign the application.

4. Mail the application or bring it to any of our convenient MAX branches.

Please note that mailed applications must be notarized.
