1. Complete all required information fields. Fields left blank may delay processing.

2. Mark which accounts you currently have with MAX.

3. Sign and date in the appointed area. If there is a joint owner on the account, they must also sign and date the application.

4. Print and sign the application. You may mail the application to MAX Credit Union, PO Box 244040, Montgomery, AL 36124 or bring it into one of our branch locations.
